In 500 words or less, please explain your motivation for pursuing admission into the program. Include any special interests, abilities or experiences which have been instrumental in your decision to seek admission, and any goals and objectives you hope to accomplish through the program.

Your creative résumé should highlight creative and artistic work accomplished, activities and relevant employment. This may include:
• Your full name and contact information.
• Any extracurricular activities or work in your community.
• Additional information such as awards, clubs, and activities.
• Links to a personal website, digital profile, social media pages, and/or links to download samples of previous work

Academic Documents (for transfer credit)

NOTE: You may submit a partial application and return later to upload these documents as they become available. Transcripts and Administrative Letter of Approval are only required for students seeking to transfer FMA credit back to their home institution at the culmination of the term.
Official transcripts from you home institution may be uploaded here or sent via via email to admissions@forummusicacademy.com. Unofficial transcripts are also acceptable.
Please upload a signed letter of approval from your dean, academic advisor or study abroad advisor.
Please submit at least one additional letter of recommendation from an advisor, professor, employer, or any other person(s) that may speak to your experience and/or proficiency in music.
